PHYSICAL ADDRESS IS CONFIDENTIAL Care Coordinators provide case management services to people living with HIV/AIDS. Care Coordinators identify clients' needs while developing a goal oriented care plan in order to meet those needs. MEDICAL SERVICES PROGRAM - Assists with application process. The program lessens the cost of medical care. HOPWA - Provides assistance with rent, food, utilities, and some mental health treatment.
