Vision screening 2:00-3:00, 5:30-6:30. Any child who will turn 5 before September 1, 2010 may register. the child and a legal guardian must be present. Please bring with you: birth certificate, Social Security card, verification of address, and custody documentation in case of divorce. You should also bring immunization records or proof of an appt. to get immunizations. (Note: the Wayne County Health Clinic will be open for walk-ins each day of the week of April 20-24 from 8:00-5:00. Wednesday of that week, the Clinic will stay open until 6:30.
