Indiana University East Psychology Co-op/Internship Application Form

This form is utilized as an interest document for Dr. Deborah Miller, Assistant Professor and the Coordinator of Applied Experiences in Psychology. Upon completion of the form, you will receive communication from Dr. Miller setting up an appointment to talk about your interest. Please fill out the information below completely and accurately.

Please submit the form according to the dates below to be considered for a Co-op experience. Co-ops are not offered in summer for Psychology credit. (Distance Education Student opportunities may need more time to process paperwork)

For Fall Semester Co-op starting August, submit application by May 1st

For Spring Semester Co-op starting January, submit application by October 15th

Please check the boxes below to indicate you have met the prerequisites required for the Co-op experience. If you do not have a prerequisite completed on the list, please submit your application and Dr. Miller will determine your eligibility.
Prerequisites Have completed 9 credit hours of advanced Psychology coursework Junior or Senior level status based on academic credits Have a 2.5 or higher cumulative GPA Must have at least 8-10 hours of available time to devote to the co-op during normal working hours.
